Screening receipt · Florida hospitals and health systems

One receipt per hospital: CCN, NPI and the Sunbiz row.

Type the exact registered name and, if you have them, the CMS Certification Number and the NPI. The receipt reads each official file or endpoint under its own key, prints what it returned with fetch date and SHA-256, and records a registry id anyone can verify. Nothing is scored or inferred; a source that did not answer is printed as such.

What the receipt reads, and under which key

CMS Care Compare (by CCN)the facility's own Medicare certification record: type, ownership as CMS prints it, ratings fields as published
HHS Medicaid Provider Spending (by NPI)sums of HHS's own T-MSIS rows 2018–2024 as billing and as servicing NPI, by year and top HCPCS — file date and SHA-256 printed
CMS Open Payments (by NPI)industry payments as CMS publishes them
NPPES (by NPI)the NPI record and how many other NPIs print the same exact name
Florida Division of Corporations (Sunbiz quarterly file)the exact registered name: document number, FEI/EIN, status, filing date, principal address — file date printed
HHS-OIG LEIE · SAM.gov exclusions · OFAC SDN · state Medicaid listsexact-name and exact-key matches in the exclusion and sanctions files we hold, each with fetch date and hash
USAspending · FAC single audits · HHS OCR · EPA ECHO · SEC EDGAR · IRSthe rest of the twenty-three-source receipt

Florida file date and HHS file date are printed on every receipt; see /methodology for each source's fetch date, row count and hash. Same receipt through the Partner API (/v1/screen?name=&ccn=&npi=) and as a saved watchlist that reports only what changed (/v1/monitor) — /partners.

Not read: officers and registered agents (in the Florida file, deliberately not loaded); anything not printed by a source. A name-only match is a printed-name match, not an identity; supply the CCN and NPI to read under exact keys.
