42 CFR 455.436 — state Medicaid agencies must check LEIE and SAM for enrolled providers, owners, agents and managing employees no less frequently than monthly; HHS-OIG Special Advisory Bulletin on the Effect of Exclusion (May 2013) — providers should screen employees and contractors against LEIE and OIG recommends monthly checks; 42 U.S.C. 1320a-7a(a)(6) — civil monetary penalty for items or services furnished by an excluded person.

Each row is one settlement HHS-OIG posted on its enforcement page, with the amount, date and employer name exactly as OIG titled it. A settlement resolves OIG's allegation that an excluded person was employed or contracted and that items or services were billed to a federal health care program; it is made without admission of liability. The dollar figures are the penalty layer only — they do not include the employer's own staff time, counsel time or repayments, and they say nothing about the far larger number of employers who screened and never appear here.
