44 states and DC each publish their own Medicaid exclusion or termination list. There is no federal file that joins them, no common format and no common key. This is what each state publishes, and which of them a program can read as a file today.
CMS State Medicaid Director Letter #09-001 (Jan 16, 2009): providers must search their state's exclusion list whenever they search the federal LEIE; CMS and HHS-OIG recommend monthly.

Rows a state added or removed between consecutive versions of its file (signature: printed name + NPI + effective date). A corrected spelling or date shows as one removed and one added. History exists only from the "tracked since" date on each row above.
Individuals without an NPI
Most individual rows on these lists carry no NPI, so the exact-key rule cannot reach them. On the single-name record only, an individual's name is also checked as a printed string: the rows whose name the state printed exactly as typed (or transposed "Last, First"), with how many rows per state share it. That is a printed-name coincidence, not an identity match; the state holds the DOB and license that would settle it, and we do not read those. It never enters a batch or saved-list count.

How long we have held each file is printed on its row. Where it says "changes tracked since" a recent date, that is how much history exists — no more. The data does not depend on how long we have been reading it; the history does.
